CASES OF HEMANGIOMA WITH MULTIPLE PHLEBOLITHS IN HEAD AND NECK REGION




Abstract
Hemangioma is the most prevalent benign tumors of the head and neck in infancy and children. But until 6-7 years old age, there are resolution of hemangioma in 80-90 % of cases except portwine hemangioma.
But, some large hemangioma masses remain without any improvement or grow much bigger. Among these long standing hemangiomas, we occasionally find the multiple phleboliths.
The presence of phleboliths in the area of body in which there are normally no large plexus of vein, indicates an abnormal collection of vascular channel which is usually cavernous type. These phleboliths are found most commonly at the periphery of the hemangioma where the stasis of blood flow is most marked.
They represent radiopaque density in roentgenogram because they consist of organized thrombi infiltrated with tricalcium phosphate or other calcium salts and are invested in a bed of endothelial cells.
We experienced 3 cases of hemangioma with multiple phleboliths in the head and neck area. The age of the patients was from 10 to 24 years old. The number of phleboliths was from 4 to 15. Two cases were cavernous hemangiomas and one case was arteriovenous malformation. We could observe many stages of phleboliths formation from the venous plugs to the white round stones during the operation.
